All eligible employees, 25 hours/week will have the following coverage through Cigna

• $25,000 Group life
• Accidental Death and Dismemberment (AD&D)
• Long-Term Disability coverage

– benefits duration to age 65
– 90 day elimination period
– 3/12 month pre-existing condition
– 60% monthly income/maximum benefit $5,000 month
– 3 month survivor benefit
